Presently serve as the nurse liaison for the entire
campus which includes a 112 bed skilled nursing facility
and rehabilitation center, as well as, a 64 bed personal
care home. In this position I am responsible to
establish patients' eligibility for care utilizing
current CMS regulations and guidelines, communicating
with families, and interacting with a wide range of staff
members, from admissions coordinators to case managers to
physicians. I am the first to assess patients, review
their medical records, and determine whether they should
be admitted for care. I explain the types of care and
services my current facility offers to patients and their
families. I also work with the facility to schedule
therapy sessions and doctor visits. Coordinating with
other staff members to ensure smooth patient discharge is
also among my duties. I work directly with the finance
department to verify health insurance and its coverage,
interacting with the providers and our facility.
Director of Infection Control and Prevention- RN

Currently serve as the RN Infection Control Nurse in a
112 bed skilled nursing facility. My primary
responsibility is to ensure an effective facility wide
program for the surveillance, prevention, and control of
infection through monitoring rates of health-care
associated infections, maintaining systems to collect and
analyze data, and organizing activities to prevent and
control infections in residents and staff. Ensure and
provide appropriate patient care through collaboration
with multi-disciplinary teams to assess, plan, provide
and evaluate patient care in a manner that maximizes safe
and effective delivery of care. Report infection data
electronically using the Pennsylvania Patient Safety
Reporting System (PA-PSRS). Oversee and delegate tasks to
all staff both nursing and non-nursing. Assist in
meeting the unique needs of every resident.
Prior to holding this position I served as both a charge
nurse and a nursing supervisor in this institution. My
responsibilities at that time included but were not
limited to: medication administration; rendering
skin/wound care treatments; assessing, reporting and
documenting relevant data on each resident and assisting
with activities of daily living.
Key Accomplishments:
Helped establish a working relationship between my
facility and area hospitals, home health services, and
hospices.
Ongoing relationship with are case managers and social
workers to ensure a smooth transition between their
facility and ours.
Helped establish and refine facility policies and
procedures in areas of infection control, vaccination and

regulations and guidelines.
Responded to and interrupted the severity of numerous
outbreaks of illness, gaining the respect of physicians,
state surveyors and coworkers for my calm quick response
to ensure safety of residents and staff.
Earned recognition from the Pennsylvania department of
health for participating in state sponsored programs to
monitor and prevent influenza like illness in health-care
facilities.
Was voted employee of the month by a group of my peers
shortly after becoming a nursing supervisor.
