I currently work as an LVN, Admission Intake Coordinator, processing new
referrals for various Hospitals, Doctors' offices and patients / families
calling independently for Home Health, Hospice and Infusion / Enteral
needs. I receive and process referrals for branches located in LA, Orange
and Riverside Counties forwarding patient information to appropriate areas,
confirming receipt and provide current status updates at the time of the
referral. I verify various types of insurance (Private, Medicare and Medi-
Cal) to obtain authorization for services to be rendered. I am responsible
for introducing patients and or their families to Heartlands program while
discussing services ordered and to verify patients' information. Contact
Doctors' offices to take verbal orders and request any missing patient
information.
I assist with scheduling patient and or family with the Admitting Team,
confirming face to face encounters between Doctors and patients have been
scheduled while confirming this interaction has taken place per Medicare
guidelines, ordering DME as needed and scheduling patient transportation to
their private residence or skilled nursing facility. All referrals are then
input into Heartlands Horizon / Vantage Point system.
I facilitate morning stand up call between the Administrators, Sales Team
and Regional Director for all branches to provide current census and
patient updates for those who came on service within the past 24 hours. I
ask the team for any updates on pending referrals. All information
collected is updated in Horizon / Vantage Point System and current
patients' records are updated accordingly.
Patients / family call backs are made weekly, bi-weekly and monthly to
patient's not currently on service or previously discharged to follow up on
their current status, information gathered is documented.
I take calls from patient / families related to hospice services listen to
what their needs are and direct / connect them with pertinent community
resource as needed.
I am responsible for in-servicing all new Account Representatives to
Heartlands Intake Admission procedures.
Harbor UCLA Medical Center 2005-2006
Worked in at a acute psychiatric facility floating to Psych ER, Psych CRU &
Psych 8 West assisting adult patients with ADLs which included disbursement
of medication, education, transcribing and entering Doctors orders into
database and making team assignments while assisting team members. My IV
certification was obtained during my tenure at this facility.
Mobile Home Extended Care 2004 - May 2005
Provided one to one skill care to pediatric patient, tending to patients'
ADLs, administrating medications, providing treatments (Trach care, G-Tube
Feedings, Wound Care & Breathing Treatments); maintain patient supplies par
levels, scheduled patients' appointments & assist Home Health Occupational
Therapist as needed.

Provide one to one skill care to teenage/young adults, assisting with
ADLSs, administering medications, providing treatments as needed, maintain
