in both small and large facilities as well as working for the large nursing

facility. In July the residents voted me as the Employee Of The Month for

the positive changes that we had implemented since my arrival there. As a

team we increased census since my arrival there. Together we also increased

our case mix score. We implemented the policies and procedures of the

Charleston Group. The residents, families and employees seem to be happier

than they were before I arrived. We also began to collect back money during

the time that I was there.

facility. On November 18, 2013 the state came in for the facility's annual

survey. They cited ten F deficiencies. Five were at the E level and five

were at the F level. They cited ten N deficiencies. Six were at the E level

and four were at the F level. Life Safety cited three K deficiencies at the

F level and four N deficiencies at the F level. The state performed a desk

review on all of the above deficiencies on January 6, 2014 and the facility

was found to be in compliance as of that date. We had three complaint

surveys which were unsubstantiated and not cited during my tenure there.

This building was recently bought by a new owner and we are working as a

team to rebuild its reputation in the community. In July the state came in

for a Change Of Ownership (CHOW) visit. They cited two F and two N tags. In

August they did a desk review of the tags and approved the CHOW. We also

called in a self report incident and the state came in and did not cite any

deficiencies. The state also came in on complaints at two different times

and did not cite any deficiencies. We began offering Medicare Part A

skilled nursing services in October. Because of census growth we have

reopened a hall which was closed. Before I arrived the facility's last

survey included K tags. In March 2013 we had survey and the highest tag

cited was an F.

I served as the Interim Administrator of this skilled nursing facility. I

began on March 20 and the facility's annual survey was from April 23 - 25.

When I left this building on May 8 we were working on the fifteen federal,

two state and four state and federal life safety code potential

noncompliance areas. As soon as we knew about these areas we developed

action plans to correct these issues and followed up with the respective

staff members on a daily basis. We were still waiting for the 2567s to

arrive. However the facility was well on its way to clear these

deficiencies when the state comes back for their follow up visit.

I served as the Interim Administrator of this skilled nursing facility. The

survey team arrived on January 23, 2012 for this skilled nursing facility's

annual survey. I became the interim administrator of this building on

January 24, 2012. When the survey team exited on January 25, 2012 they

cited twelve F tags including an SQC, five K tags and twenty N tags. When

they came back for the follow up survey on March 1, 2012 all deficiencies

were cleared during my administration. The state also came in on two

complaints and a self reportable while I was there and did not substantiate

and/or cite any deficiencies on these matters. This was all accomplished in

such a short period of time by working closely with our company

consultants, department heads and employees as a team.

to 93 residents. We also lowered our accounts receivables. When I left the

This was done by working closely on a regular basis with the department

heads and company consultants. In March 2010 this facility received no

deficiencies on their annual survey. In January 2009 before I arrived, this

facility received 12 deficiencies on their annual survey.

staff we cleared all 43 deficiencies on our follow-up survey by the state

on January 10, 2007. I served as a Preceptor for two Administrators In

Training in this facility. On our annual survey in April 2007 we reduced

our deficiencies to 8 from the previous 43. On June 26, 2007 the state

cleared us of all of the 8 deficiencies from our April survey. On our
