PROFESSIONAL EXPERIENCE:
September 2013 – Present Staff RN, Covenant Home of Chicago, an
assisted living facility with over 45 residents. Strengths in
communicating with older adults with dementia, identifying
psychosocial needs and integrating them into overall care
plan, communicating and coordinating care needs effectively
with other team members and outside clinicians, astute and
thorough assessment writing, and taking initiatives to
improve care. Based on observed patterns, initiated an
evidence-based study to identify and reduce risk for falls
related to postprandial hypotension (after-meal BP
fluctuation) in adults over age 75. Led in-service training for
CNAs to improve knowledge and safety surrounding oxygen
delivery equipment.

visual analysis, critical thinking and accurate communication
related to objects and their surrounding context. Promoted
