Objective

Experience
Home Health – I am currently employed with a local agency where my attention to
detail and ability to multitask efficiently are of utmost importance. The
responsibilities with this position are quite varied and include scheduling all visits,
review of frequency to ensure care plan is implemented appropriately and within
federal guidelines. Additionally, I participate in the intake of new referrals,
insurance verification, obtaining necessary documentation for admission.
Performing quality assurance audits. I assist case managers by receiving and
processing physician orders, all laboratory results and managing the Coumadin
Clinic. Maintaining multiple logs such as hospital log, referrals, emergency
preparedness records and census spreadsheet. In this position, coordination
between physicians, field staff, administration, patients and caregivers is essential
to ensure the highest level of care is provided, in a safe and effective manner. This
requires excellent organizational and coordination skills via written, electronic and
phone communication.
Physicians Clinic – I worked for local cardiologist and was responsible for many
duties in providing care in a clinic setting. These duties included phone triage,
patient assessment prior to physician evaluations, performing EKG, patient
education, prescriptions, scheduling and working in Coumadin Clinic performing
PT w/INR and adjusting dosage. Also, I was responsible for scheduling
physician’s outpatient surgical procedures and providing appropriate
documentation to support authorization. Additional responsibilities included
making all necessary referrals to ancillary services, DME equipment, medication
assistance and providing all necessary documentation to supports patients’ need
when deemed disabled.
Education
Lamar State College – Port Arthur, TX, Graduated from the nursing program and
subsequently obtained my LVN license in 1995.
Lamar University – Beaumont, TX, Attended prior to enrollment in nursing
program and have attended since in pursuit of furthering education.
