mental or physical illness, poverty, unemployment or physical abuse. Interview clients, review

records, conduct psychosocial assessments, or confer with other professionals to evaluate the

mental or physical condition of clients or patients. Collaborate with counselors, physicians, or

nurses to plan or coordinate treatment, drawing on social work experience and patient needs.

Agency. Responsible for providing access to housing options and assist in triaging individuals to

appropriate housing and mental health needs. Assess clients’ needs, strengths, and gaps in

consist of all ages with various injuries. My responsibilities consisted of attending medical
