mental or physical illness, poverty, unemployment or physical abuse. Interview clients, review
records, conduct psychosocial assessments, or confer with other professionals to evaluate the
mental or physical condition of clients or patients. Collaborate with counselors, physicians, or
nurses to plan or coordinate treatment, drawing on social work experience and patient needs.

Responsible for providing service to individuals involved in a motor vehicle accident. Clients
consist of all ages with various injuries. My responsibilities consisted of attending medical
appointments, arranging transportation, obtaining physician orders and carrying out orders as
prescribed, including arranging for durable medical equipment, obtaining medication and
scheduling medical procedure and documentation of contacts with clients during appointments.

placement and special education. Initial home visits and subsequent visits to new clients were
