RNAC: Conestoga View Nursing and Rehabilitation 2013-2012

Accurately, thoroughly, and promptly completed the MDS assessments. Identified changes in the level of resident’s care

and based on the changes, revised and prioritized the completion of assessments based on the RUG that could be

obtained and therefore increasing the CMI for the facility. Researched and procured data from various patient records as

well as interviews of residents, families, and staff to initiate the most detailed and accurate plan of care for the resident.

Oversaw the nursing process of a staff of 12 healthcare professionals. Managed the delivery of care

to a population of 149 geriatric residents, using the nursing practice as dictated by professional

licensure of the state of PA. Directly reported changes in resident’s status to PCP assigned to care of

the resident. Utilized data accumulated through assessment as dictated by professional standards of

practice. Implemented orders received by PCP. Evaluated resident’s responses to plan of care with

revision and further follow through when deemed necessary. Communicated with administrator, DON,

ADON, and other management professionals when situations presented themselves appropriate with

clear, concise, and professional demeanor and conduct. Facilitated and performed a daily audit of the

delivery of care on every unit and documented on a supervisory communication report any data

missed on admission, any follow-through or documentation missed on incident reports and

consultations, and any aspects of care that were not followed up on during the day. Daily audits of

care delivered over a 24 hour period to facilitate continuity of care. Addressed staffing issues. Daily

audit of plan of care over a 24 hour period and reported follow through missed in an effort to improve
