Responsibilities
I started work in the nursing home dept. as a staff nurse. I assessed residents, administered medication and
documented care provided. I was promoted to wound care/restorative nurse. For residents with wounds I
frequently assessed/documented wounds and was a liaison with MD for proper wound treatment as wound
progressed. I also educated staff on wound prevention. As a restorative nurse I coordinated care for residents
who qualified for restorative program. I assessed residents and implemented care plans for the residents who
would benefit from the program.
Accomplishments
We had a decrease of residents with wounds while I was in this position. I assisted the DON with the start up
of the restorative program and because of the restorative program we had an increase in case mix score.

I demonstrated good leadership and teamwork skills. I encouraged all staff to participate in wound prevention
and restorative care.

Responsibilities
I started work with this home health company as a field nurse assessing patients and completing oasis for
admission, recertification, and follow up of home bound patients and provided skilled care to these patients. I
was promoted to branch manager after a year with this company. As manager I oversaw and supported staff
in patient care activities and office staff in all clerical duties. I processed monthly reports to my supervisor,
maintained budget, ensured HIPAA compliance, maintained supplies, scheduled visits, and provided all other
manager responsibilities.
Accomplishments
I created a work atmosphere of not only morality and professionalism but also friendly and fun. Staff frequently
stated how they loved their job and loved their patients. They stated they actually wanted to get up and come
to work each day. That made me feel good about what I was doing. Our office always maintained a high
case mix budget and received high scores in chart audits and patient surveys. We was accredited by Joint
Commission during my management after a surveyor audited our office. I had many great accomplishments
and experiences during my work here.
Skills Used
I learned and grew so much from my experience with this job. I learned great leadership and management
skills and went to leadership workshops. I also improved my teamwork and communication skills. Due to the
extensive documentation required for home health I have very good documentation and computer skills.

Provided Nursing Care to med/surg patients. Closely monitored acute conditions medically and after surgery.
