and primary focus was to make sure my participants were safe living in the
community. I did home safety checks every 6 months. I performed semiannual
and annual assessments physically and home assessment to assess if they
remain eligible for our program. I was responsible for all their care that
was delivered to them, as far as home health care, medications and
education in the home for caregivers or participants. I did med boxes if
need and educated them and families on how to care for them safely at home.
We have a team of doctors and NP to coordinate their care. I would do home
visits if they were sick, had a fall, needed injections or Foley catheter
insertions. As a homecare nurse I was an advocate for them and their
families and kept them safe. I assessed if they were declines in function
and saw to it that therapy was ordered to maintain them to their maximal

Job Description: I am currently responsible for staffing of the
clinic. I oversee the care of 303 participants and there care at 2 clinic
sites. I have a Eastside and Westside clinic to staff and oversee the
function and well being of the care provided to the participants. I used
multi tasking skills and critical thinking to perform my duties daily. I'm
also very hands on with blood draws, patient care, wound care and

Job Description: I was hired as admissions and wound care nurse and
performed that task for approximately 4 months. I was responsible for
weekly daily and weekly wound care for the entire facility wounds. I
measured and documented wounds weekly and responsible for reporting to the
doctors and NP the decline or progression of all wounds in facility. I
reported any new wounds and obtained orders for them to start treatments.
Then promoted to 1st floor Unit Manager, in which I was responsible for all
3 shifts of staff and my residents on the 1st floor. I was responsible for
overseeing of the care for my resident and making sure all orders were

report to the DON any issues, but I was foremost to problem solve and multi

I performed my nursing duties as a Floor nurse working on the Rehab unit. I
was responsible for direct care of the patients, overseeing the STNA's and
their assignment to best care for the patient and coordinate their care on

status of patients to MD, NP and the DON immediately. I also performed IV

also very closely with the DON to care for the patients daily. I followed
on orders from MD's and NP's with the nurses and STNA's to make sure all
care was being carried out as ordered. I oversaw the daily assignments,
attended IDT meetings with the team and families to set care plans for
patients and better care for them as a team.
