As a Masters level mental health clinician working in Gould Farm’s (located in Monterey, MA) transitional, residential Boston Area Program, provided
therapeutic support and coaching of ADLs (activities of daily living) through weekly case management meetings to a case load of 8- 13 individuals;
mostly with schizophrenia, bipolar disorder and major depression. Several clients were dual diagnosis and ongoing relapse prevention strategies were
also provided. Worked with several clients for 3-5 years, meeting weekly, and performed monthly home visits to clients in our Extended Community
for those who had moved out of our residential level of care. All contacts documented and regular treatment plans completed. Areas of focus in this
client- centered, community-based program include: support in obtaining volunteer or paid work, starting/resuming college, finding housing,
cohabitation communication skills, co-occurring disorders/dual diagnosis, DBT/CBT, motivational interviewing, substance abuse/addictions, relapse
prevention, stress management, self-esteem and meditation.

Primary duties included conducting intake/needs assessments of offenders upon their release from prison and developing and implementing service/
treatment plans for clients. Facilitation and follow through on referrals for housing, medical and dental care, MassHealth, substance abuse counseling,
rehabilitative support groups and any other Span services or referrals that could assist clients in their acclimation back to mainstream. In addition, I co-

assessment form. In-service trainings and conferences included sessions on: ADHD, HIV and correctional facilities, smoking cessation, SMART
