Provide mental health services to participants in multiple East Bay locations. Services include clinical assessments, treatment planning, individual therapy and rehabilitation sessions, collateral contacts, referrals, and case management. The PSC is responsible for working as part of a team and provides direct services to participants as well as collaborates with other agencies and professionals.

Job Title: Personal Services Coordinator

Principal Duties and Responsibilities

1.Responsible for the assessment of Severe and Persistently Mentally Ill adults and development of an individualized, collaborative, strengths-based care plan for each assigned participant.

2.Maintain appropriate case documentation per Medi-Cal and other contract guidelines utilizing Alameda County’s Electronic Medical Record system, Clinician’s Gateway.

3.Provide individual rehabilitation and/or therapy services in the office and community based settings.

4.Work to meet participant’s needs in the areas of: activities of daily living, household management, financial literacy, budgeting, money management, banking, safety awareness, access and use of community resources, entitlements and public assistance, access to medical, dental and wellness services, mental health medication and illness self-management, which involves developing a plan with the resident to help monitor their symptoms, track early warning signs, develop coping skills and prepare a plan to minimize and prevent relapse.

5.Provide crisis intervention which may involve Mobile Crisis Team, police or other emergency workers.

Job Title: Mental Health Clinician

Provide counseling and education services to residents of Cronin House; Facilitates educational groups, family groups, process groups, aftercare groups, and individual sessions. Also participates in the intake process and chart Medi-Cal notes.

Principal Duties and Responsibilities

1.To provide group and individual counseling to clients with Co-Occurring Disorders and their families

2.To provide case management services as needed including but not limited to the following:
