Case management for a homeless or at risk of homelessness population. Case managed a caseload of 17 client from 2 different program. The first program was HAPP Housing Assistance Pilot Program through the city of Anaheim for homelessness prevention through McKinney-Vento funding. The second program HSP Housing Support Program was with families referred through Cal- Works from social services administration. Duties included outreach to clients in reference to any other assistance needed, scheduling of at home or in office visit for monthly case evaluation sheets as well as monthly budget sheets and client care plans. Enrollment of new clients for HAPP or HSP. Referrals as needed client for services such as mental health, immigration and child and family stabilization. Data entry into HMIS systems for client tracking and data storage as well as case notes for every client. Weekly supervision meetings and running of the board bi-weekly meetings to go over updates on client caseloads.

Responsibilities included supervision of 6 residents from ages 13 to 17 years old. Other responsibilities included setting up activities to teach self-help skills such as doing laundry, hygiene, social skills, emotional development as well as anger management skills. Counseling was also included for high risk behaviors such as running away, drug use, and many other behaviors.

Responsibilities included child care services for participants receiving behavioral health services. Participants had to be assessed before seeing therapist for the first time. Assessments along with appointment notes and any other paperwork had to be filed within 48 hours of being received. Telephone etiquette was a must when dealing with participants and confidentiality had to always be respected when it came to participant information. Weekly compliance reports were a must for participant files and had to be kept up to date. Time management was a must as important data had to be turned in within a timely manner.

Responsibilities were shared with the preschool teacher. The classroom had 12-17 preschool children in the classroom. Playground as well as classroom and restroom supervision.
