Primary duties include visiting patients in the their home or other residential setting to assist in provision of long term care. evaluation, treatment, and case management for children birth to three years old who have or may have developmental disabilities as identified by IDEA ( Individuals with Disabilities Act). Primary duties include obtaining medical histories by reviewing medical records and conducting evaluations for purposes of eligibility for the program. Evaluations made regarding health status, growth, vision and hearing screenings, nutrition, and immunizations. Served as health liaison with child’s family, treatment team members, and medical community. Provided consultation and technical assistance to families, agency, and private providers involved in the care and treatment of the child. Documentation, assessments, interpretation of findings, and case management are also regularly required duties of this position.
