Employment History

Canna Care Docs- January 1, 2018 to Present. Canna Care is a contracted company by the state to provide certification opportunity and education to qualifying patients in the State of Connecticut for Medical Marijuana (MM).

Current role as an Independent Nurse Practitioner is review of medical documentation presented by the patient to ensure criteria for certifiable diagnosis is met. In conjunction, eligibility requirements for Medical Marijuana based on the DSM V (2013) diagnostic criteria for Post-Traumatic Stress Disorder (PTSD), are implemented to make the diagnosis of PTSD. Once the criteria are met, the patient is then educated regarding the physiology of the diagnosis, how and which symptoms Medical Marijuana may provide relief and which symptoms require Trauma and PTSD focused therapy to alleviate.

Health Fair- August, 2017 to January, 2018 (seasonal contract). Health Fair is a mobile clinic contracted by private and public health insurance groups. The goal is population risk stratification for specific chronic conditions based on subjective and objective data gathered on one annual visit. The findings are streamlined into one system and submitted back to the contractor.

The role of the Nurse Practitioner in the clinic is to assess results of blood work, vital signs, subjective and objective data and active medications from that day only. Critical evaluation of obtained patient information can result in new diagnoses, urgent and routine referrals, screening test orders and recommendations, education, 12 lead EKG review, mental health screening and review and collaboration with patients outside providers as necessary.

Connecticut Mental Health Specialists (CMHS)- February, 2017 to June, 2017. Behavioral Health Consultant for residents in nursing homes, assisted living facilities and rehab centers.

Assess, diagnose, treat, follow up and educate patients with established and new mental health conditions who reside within these settings. Collaborate with other members of the healthcare team regarding possible underlying medical conditions contributing to behavior changes. Prescribe psychotropic medication when necessary, obtain necessary consent, involve family members when establishing a patient plan when appropriate, monitor patient for medication interactions, adverse effects, therapeutic effects and toxicities. Eastern Connecticut Health Network (ECHN)- June, 2009 to December, 2016. Gastroenterology (GI) outpatient clinic with Dr. Peter Buch and Dr. James O'Brien. Consult and follow up with patients in the office setting for various GI symptoms and diagnoses. Assess, diagnostics and labs, diagnose, educate, collaborate, plan, implement, follow up and plan revision as necessary. Order medications, implement holistic adjuncts to therapy if necessary and communicate with insurance and pharmaceutical companies on the patients behalf. Bill according to ICD 10 coding standards. Utilize EMR for medical chart review and current documentation.

ECHN-Sept, 2011 to January, 2015. Gastroenterology (GI) Inpatient NP consultant at Rockville and Manchester Hospital. Weekend service coverage for new consults with suspected acute GI related issues and follow up visits for hospitalized patients under the GI service.
