Skills and Work Experience
Halcyon Rehab at Wave Crest: (OTR) October 2016-Present. This is a SNF facility with LTC, the rehab focus is on bundle Part A, HMOs, part B, and Medicaid. This is a PRN position but has been the main OTR for the Occupational Therapy caseload and supervising the COTAs; other responsibilities include writing RNP/FMPs, patient/caregiver education, d/c planning and screening referrals. We have a diverse population of geriatric, middle/young adults and psych residents including rehab setting.
Rockledge Health and Rehab: (OTR PRN) August 2016-Present. This is a skilled nursing facility; evaluating and treating patients; Medicare A, HMO, Part B skilled services.
Life Care Center Melbourne: (OTR) February 2015 to present; Currently a full-time OTR; Prior to employment completed a 12 week OTR rotation from September to December in 2014.Until I received OTR license I worked PRN in this facility for a year and a half as a COTA/L. This is a skilled nursing facility & Rehab/Outpatient setting with the geriatric/adult population. This rotation allowed experience in home assessments prior to patients discharging to their home as well.
Melbourne Terrace Rehabilitation Center: (OT 12 Week Fieldwork Rotation) January 2016 to March 2016. This is a Rehab/Outpatient setting with the geriatric/adult population. This rotation allowed experience with aquatic therapy, working closely with the stroke team and the ability to observe lymphedema treatments.
Brookdale Senior Living: (COTA/L full-time then PRN) January 2010- March 2016 as a fulltime COTA/L and assistant manager until April 2015 became PRN. This is an assisted living facility for outpatient and the home health community treating the geriatric and dementia population. Responsibilities include scheduling part B caseload, chart auditing, receiving consents, screens, and insurance verification, home health treatments. Nurse on Call also merged into this company and part of the Occupational Therapy department at the West Melbourne, Greensboro buildings.
Genesis Rehab: (COTA/L part-time).April 2007- January 2016.The patients treated in this location were for outpatient, sub-acute, long term care, respiratory/trach mgmt., or dementia programs in various local rehab sites such as skilled nursing and assisted living facilities with in the local community including participation in Car Fit program. The facilities worked in, were previously La Vie and Ambassador companies and became Genesis in 2012.
Alliance Care: (Patient Care Manager and COTA/L.) November 2004-June2005.Provided rehab management and treated patients in assisted living communities locally at Village Oaks of Melbourne and assisted with two other communities until the rehab company was sold. Services provided were home health and outpatient, chart auditing, screens, insurance verifications, receiving consents, training ADLs to the home health aides, team meetings, training sessions and managing the rehab caseload.
Circles of Care: (COTA PRN and field work rotation).February 2003-November 2005. Occupational therapy based therapeutic groups with clients of all ages coping with mental health, substance abuse, domestic violence and dementias. Some of the groups implemented were ADL retraining, reminiscing, physical activity, art activities, food preparation, teamwork, assertiveness, community resources, safety, self-esteem, problem solving, etc. I also ran two groups a week on the children’s unit.
Atlantic Orthopedics: (COTA/L Fieldwork rotation) 2003. Hand specialty orthopedic clinic, wound care, splinting, modalities for outpatient injuries & recoveries from surgeries. Clients are seen for workman’s compensation cases, part B services & other insurances.
