Current Position

I am applying for your position. I have a Bachelor’s Degree in Psychology and a Masters in Social Work. I am a LMSW and certified Christian Counselor. For over 16 years I have worked in Social work and Case management for the State of Texas, private sector and in a non-profit in 24 local counties. My primary focus through the years was to perform assessment of the resident at admission, upon change of condition and/or annually; create, review and update care plan and progress notes, psychosocial intervention, help patients and family in coping with placement in a skilled nursing facility, physical illness, disabilities of the resident, and the grieving process, coordinate resident visits with outside services. I have facilitated meetings for interdisciplinary teams over 6 years in case management in clinical setting along with providing individual and group counseling to clients with a specialty in Mental health and children and families. I have worked with every population of people in multiple settings.

I have worked in a Social Work and Person oriented Setting since I was 14 years of age. I believe with my experience I would be a good fit for your position. I very familiar with networking and building relationships, with other

employees, Law Enforcement, County/District Courts, Doctors/Medical staff,

Home Health Agencies and DADS. In a fast paced day I will wear many hats:

Diagnostician, investigator, advocate, counselor, and mediator. Health is

ever changing system from insurance, computer systems, policy, and

paperwork. Due to this I am a fast learner, and I can change quickly when

needed. I have prepared legal binding documents, care plans, continuing

narratives on behaviors and medical documentation, and counseling

sessions, as well as monitoring visits and progress from state housed and in

home clients. I am a people person. In my free time I have worked in multiple

ministry positions all-encompassing people and providing for their overall

Complete needs and discharge planning for Pt. Complete Adult Psycho/social assessment Perform assessment of the resident at admission, upon change of condition quarterly, yearly and on request. Provide training for staff and residents for rights and abuse and neglect. Complete all grievances and abuse and neglect self- reporting. ABSSLC ABILENE STATE SUPPORTED LIVING CENTER COUNSELING AND DESINITZATION INTERN PROGRAM PROVIDER 9-15-14 TO 9/22/15

Completed group and individual Cognitive Based Counseling to IDD individuals at a ICF from behavioral management, anger management and task management and social skills. Coordinate resident visits with outside services -dental, optical, etc. Work with the resident, family and other team members to plan future education or to help discharge back into the community. Conduct in-service programs to educate staff regarding psychosocial issues and resident rights. Supervise and guide Social Services Assistants. Attend and document resident council meetings. Participates in departmental budget planning and supplies monthly report to Administrator. Perform additional duties as assigned. I also provided Dental Densitization to 30 individuals twice weekly to lesson fear and build safety systems to keep from restraints and general anesthesia. Helped to build assessment tools to provide these services according to federal standards of Dentistry and Psychology.

DSP live-in staff providing all ADLS assistance needed 24/7 and transport to and from day hab. I completed daily trainings in mobility, health care, diet, medication and basic learning skills for life. I took this job so I could work while going to school fulltime for my Master’s in Social work. I then fell in love and got married and since it was a live in job I had to quit.

DIVERSIFIED WELL LOGGING-MUDD LOGGER-2-11-13 TO 7-13 MIDLAND, TX

Mudd logger testing soils and monitoring gases.

Completed set up and take down of work trailer and gas monitoring systems. Completed gas monitoring of H2S, Propane, Butane, Iso-butane, and Methane. Tested soil for safety and drilling concerns every 10=90 feet. Keep comprehensive logs of all safety and drilling concerns 12 hrs. 7 days a week from beginning and end of well. This was a deviation from social work to put back funding to prepare to pay for my education.

Needing to have time for school and needing extra income I have worked for months at nights at Wal-Mart. This allowed me a physical job while allowing my days to use my mental abilities for school.

During my time with DFPS I held a case load of anywhere from 15-50

cases a month. I work as an investigator in the first line interacting

with families and disabled adults. I worked simple cases of Physical

Neglect: teaching families and adults basic hygiene, budgeting, medication

management and parenting skills. As my skills progressed I worked as the

Senior Investigator in my office working the most severe cases of sexual

abuse, physical abuse, and child death cases.

1991-Present Volunteer Work: By the age of 12 I felt I had discovered my calling working with Children’s Church. Today I am still involved with our children’s church, Youth and The Women’s Ministry. I see the value of every person. I have a desire to help the undesirable people. I work with people others might not consider worth the effort. Simply because I believe some of my greatest work has been from my Volunteer work work that I never receive monetary recompense for but receive my pay in seeing those same lives changed.
