SUMMARY:

As a Rehabilitation Case Manager I provided adults and older adult’s rehabilitation and restorative skill based services as well as case management and referral services, and preparation for independent living and/or improved self-sufficiency. I also provided counseling services utilizing the Transformation Care Planning model to improve client’s emotional wellbeing and life domain functioning.
