SOCIAL SERVICES EXPERIENCE

OU Physicians College of Medicine, Tulsa, Oklahoma Department of Family Medicine, June 2004 to June 2017 Social Services Specialist - Interviews patients and family members to obtain information about psycho-social needs, home environment, patient and relationships and continuity of care. My responsibilities also included developing a case plan to assist patients and families with social and environmental difficulties that result from illness that may interfere with medical treatment. Documents patient contact in patient’s medical record. Collaborates with and assumes coordination responsibility with case managers, managed care, physicians, nurses, community agencies and insurance to provide comprehensive social services to families in need. Responsible for filling out appropriate paperwork associated with patients, works with physicians to meet patient’s needs. Juvenile Bureau of the District Court, Tulsa, Oklahoma January 1997 to June 2004, Supervisor: Patrick Obrien M.S., Education Probation Counselor – Conducts investigations regarding the juveniles past, including school records, arrest records, arranges psychological testing, medical examination, complies social histories on each individual client. Accompanies juveniles to court with prepared services plan and recommendation. Implements counseling plan, work in conjunction with law enforcement personnel, school officials, and other divisions of the Bureau. Performs case work, and related service for youth and families; develops individual treatment plans for youth on probation and their families.
