JOB DESCRIPTION

Principal purpose of the position:

Provides and ensure that all patient’s / clients of the nursing unit are provided with direct hands on quality patient’s care that conforms to the standard of the hospital.

Duties and Responsibilities:

Assess nursing care needs to patients and problems of patients assigned to me.

Assess patient’s needs by: collecting data about the patient, reviewing the clinical records knowing the medical diagnosis and positively seeking information from the physician and other personnel caring for the patient.

Creates plan of care for assigned group of patients based on the assessed needs.

Records patient’s needs and problems and writes appropriate nursing directives for activities, techniques, equipment or supportive measure in the Nursing Care Plan (NCP) and patients chart.

Coordinate the medical care plan with the NCP, interpreting doctor’s order to achieve the most effective result, relates doctor’s orders to appropriate nursing measures. Assumes responsibility for the execution of the doctor’s orders. Solicits patient’s cooperation in planning and implementing plan of care.

Makes rounds alone and with physicians; plans patient care with the doctor and the patient, and communicate patient needs and problems to the doctor as necessary.

Encourage and utilizes reported observations and suggestiong of all levels of nursing personnel in planning nursing care of patient.

Give recognition to the real and potential contribution of family and significant others in planning patient care.

Assist other registered nurse, aides and technician in following through with all informations and directives of nursing care plan.

Supervise and directs activities of nursing aides.

Assumes responsibility of nursing care unable to performed by the nursing aides.

Gives brief report to Head Nurse or Charge Nurse at the end of shift for continuity of patient care.

Listen to, evaluates and act on needs, problems and suggestion of patient’s visitors, doctors and others.

Check own documentation for completeness, pertinence, evidence of give and record of patient progress.

Evaluates effectiveness of care plan, comfort and safety of nursing care and procedures performed.

Evaluates the degree to which patients needs and problems are being met.

Charge Nurse: assumes the duties and responsibilities of the Nurse Unit Manager.

As a Unit Clinical Educator: orients and teaches Nurse Trainee/s assigned in the unit.
