In connection with your application for employment, reassignment or promotion with Aerotek, you were provided notice that a background check would be performed post offer as part of the Company’s Employment Selection Process. You also completed and signed an Authorization for the Procurement of a Consumer Report Form to enable the Company to secure the applicable background report from its authorized third party vendor. In accordance with its Employment Selection Policy, the Company conducts a targeted screen of all background report results. Based upon an initial screen of your background report, you may be disqualified from further consideration for the position for which you applied due to Misdemeanor Criminal Mischief. Accordingly, this letter is to inform you of the following: (1) that the Company is considering making an employment decision based in whole or in part on the Company’s initial screen of your background report; and (2) that the Company is offering you the opportunity to participate in an individualized assessment process to determine your continued employment eligibility for the position. To that end, we provide you with the attached information: a) A copy of the background report;
b) A Summary of Your Rights Under the Fair Credit Reporting Act, as prescribed by the Consumer Financial Protection Bureau; and
c) A Background Individualized Assessment Form for your completion. Please review the enclosed copy of your background report and the Summary of Your Rights Under the Fair Credit Reporting Act. If you believe there are inaccuracies, incomplete information, or if you dispute any of the information reported in the background report, you need to contact Sterling Backcheck within five (5) business days from the date of this letter at:

In addition, if you would like to participate in an individualized assessment by providing additional relevant information concerning the facts or circumstances surrounding the reported information for the Company to consider, you must complete and return the attached Background Individualized Assessment Form by email or fax within ten (10) business days from the date of this letter to: ******.*******@**********.*** or fax to 646-***-****. 2
The Company will only consider additional information provided on or attached to the enclosed Background Individualized Assessment Form. Please be advised, if you fail to timely return the Background Individualized Assessment Form, the Company will render a decision based solely on the information obtained in the background report. Any and all information obtained during the Company’s Employment Section Process will be treated as confidential and only used for its intended purpose.
If an adverse decision affecting your employment occurs based in whole or in part on the information contained in the background report, you will receive notice by mail. Please be advised, if the Company ultimately determines that you are disqualified from further consideration for the position in question, you remain eligible to apply and be considered for other positions with the Company for which you believe you are qualified.
