Job Discription:
A caregiver is a person who assists the elderly by performing a number of tasks; a Caregiver
provides compassion, care and assistance to those who need help in everyday living.
Primary Duties and responsibilities of a Caregiver:
Personal Care Assistance
• Assist in shower and personal hygiene.
• Provide personal grooming, nails, hair and oral care.
• If the patient is in the wheelchair - transfer her/him from bed to wheelchair.
• Toileting assistance and incontinent care.
• Encourage activities in accordance to patient's needs and capabilities.
• Maintain a professional image and demonstrate commitment in personal growth.
Health Care
• Make an appointment if patient's needs to see a doctor.
• Report of sudden changes of the patient's physical mental or emotional health to his/her
family or if you’re in a retirement home, report it to the nurse on duty.
• Provide good skin care and avoid bed sores.
• Give the right medication according to doctor prescription.
• Monitoring of vital signs, blood pressure, pulse, temperature and respiration
• Documentation of the client's care in appropriate forms
• Reminding of prescribed and/or over the counter medication/medications
• Reporting of sudden changes of the client's physical, mental or emotional health to the
agency and to His or Her family
• Provide good skin care and avoid bed sores
Household Management
• Maintain the cleanliness of the patient's house or apartment.
• Make the patient bed and change the bed sheet regularly.
• Wash and dry the patient's clothes regularly.
• Wash the dishes and all kitchen utensils.
• Do food shopping and etc.

Changes or Things to Be Aware Off
• If your patient's has a high blood pressure - Be sure to check her/his blood pressure
regularly and report it quickly if there is high and low changes.
• Changes in Blood Sugar
• If the patient falls, Swelling part of the body, Vomiting, Confusion and Hallucination,
Changes in blood pressure (Too high or Too low), Changes in blood sugar (Too high or
Too low), If the client falls, Burn, Skin tear, Redness on skin, Weakness, Vomiting,
Bleeding, Diarrhea

• Assists in serving and feeding.
• Use safety measures for patients and self when using body mechanics, side-rails,
collapsible tubs, etc.
• Assists with restorative nursing procedures: good body alignment, bed positioning, range
of motion exercise, transferring patients.
• Assists the client to the bathroom as necessary.
• Attends scheduled therapy sessions for better understanding of a client's needs and
progress.
• Encourages and assists the client to attend all therapies and programs ordered by the
doctor.
• Plans and organizes care so that patients can attend activities or therapies as scheduled.
• Observes and reports unusual symptoms, changes, accidents and injuries.
• Make beds and clean room on a daily basis.
• Maintains good standards of cleanliness and grooming.
• Must have good physical health and be willing to be on her/his feet during the entire
work shift.
• Participates in patient care conferences.
• Dressing assistance -- Can be 15 minutes in morning and/or evening.
• Personal grooming – Brushing teeth, washing, shaving, fingernail and toenail care, etc.
• Bathing and shampoo – Assisting into the shower or bath, and/or actual help with
bathing. The caregiver is usually present for the entire bathing activity and does not leave
the patient alone. Specify how many times per week.
• Transferring - From bed, chair, toilet, into vehicle, etc.
• Toileting - Various levels of assistance may be needed for this issue. Cleanliness and
dryness are important not only for physical comfort but also for overall hygiene to
prevent spreading bacteria. Specifics should be detailed with the caregiver.
• Medication – All medication assistance must be administered with strict adherence to
doctors’ orders. No changes should be made without family and doctor approval.
However, help can be as simple as assuring medication is taken, to preparation, to actual
administration.
• Meal preparation and serving – Which meals and how many times per week. A nutritious
menu must be discussed and approved, consistent with the patient’s dietary requirements.
• Mail and newspaper – Brought in daily. Decisions should be made regarding who will
read and deal with the mail if the patient has vision or cognitive impairment.
• Housekeeping – Caregiver duties can include cleaning, making beds, doing dishes,
laundry, ironing as needed, cleaning kitchen and bathroom, and especially maintaining
sanitation, which includes regular garbage removal.
• Transportation – To and from appointments, stores, entertainment, adult day care.
• Errands and shopping – The patient may be able to go out with escort assistance, or
errands may need to be done by the caregiver.
• Ambulation assistance – Whether the patient is able to walk or is confined to a
wheelchair, it is important to exercise regularly. The doctor may have given guidelines
for exercise, so discuss the type of walking or exercise needed and how frequently.
• Companionship – Assisting with attendance at social events, escorting to adult day care,
or the caregiver provides social activities such as reading aloud to the patient, playing
games or doing crafts and hobbies.
• Management of symptoms – If the patient suffers from an illness that causes pain, rashes,
lesions, swelling, breathing difficulties, etc., the doctor’s recommendations for
procedures must be discussed.
• Therapy – Either occupational or physical. This may be part of elderly home care, or the
patient may go to an outside physical therapy facility. Some continuous care campuses
have physical therapy on site.
• Emergency service – Important contact information is needed for both a senior home care
service and elderly long-term care facility. Make sure you have an emergency contact
sheet hanging on the refrigerator that lists the family contact, any serious elderly
problems, primary care doctor, any specialists, hospital, pharmacy, etc. Discuss with the
senior home care service whether the caregiver will administer CPR before or after
dialing 911/ any emergency number.
• Medical care – Caregiver duties provided by an RN or LPN, (or else staff in the facility)
depending on the circumstances and as recommended by a doctor.
• Hospice care – Hospice care is usually in duration of 6 months or less, to keep patient
physically and emotionally comfortable during transition period.
• Communicating – Both progress and any issues should be communicated by a healthcare
service with the family.
