Hospice Care Centered Approach: Team approach and comfort patient/ family centered care;
coordination of patient/ family, provider, behavioral health, volunteers, pharmacy, home
health aide, social worker, chaplain, therapist, RN to provide medical, psychological and
spiritual support. Training and education of nursing role to support respectful skilled care to
assess, listen to concerns, contact provider and pharmacy, medication reconciliation patient
and family health education and care coordination.

Executive Officer’s mission and values for patient centered care.
Facilitate and attend diverse committee meetings providing detailed data and up-to-date
