Noc Shift R N Supervisor : Currently I function as the Noc shift supervisor for an
A lzheimer’s and Dementia facility. I am responsible for overseeing 140 bed care center,
a long with supervising LVN’s, CNA’s and other unlicensed personnel. I do admissions,
d ischarges; work with Hospice, Social Workers and Therapists to maintain the continuum of
care. I implement care plans along with following the policies and procedures of my facility.
I facilitate the care between the Doctors and Dentists that visit the residents. I also utilize

Home Health RN : Working for a family with an adult daughter with MS. Position was
total care with all ADL’s. Worked as an individual case manager, arranged all care
i ncluding physical, medical, and mental health care, and was responsible for all
t ransportation. Was a liaison for the family between physicians, nurses, social services as
