Analysing and reviewing the clinical charts of the patients
Belonging to Surgical, Medical and Gynae/Obst and Emergency
departments and its Sub Speciality both of Surgery and Medicine
Surgery ICU,and MICU .Neurosurgery,Cardiosurgery,Abdominal
Surgery like Cholecystectomy,Herniorraphy,Appendenctomy,Total
Abdominal Hysterectomy CPR when need arises Awaring the Doctors
about the stay of the patient in the hospital according the
INTERQUAL LEVEL OF CARE CRITERIA.
For Acute Adult Criteria.Patients with
O BSERVAT ION STATUS(6-24
H OURS) does not meet Acute Care Criteria and any
of the following
a pply:
• Diagnosis,treatment,stabilization and discharge can reasonably be
expected 24 hours.
• Treatment and/procedure will require more than 6 hours observation.
• The clinical condition is changing and a discharge decision is expected
within 24 hours.
• Unsafe for patients to return home/caregiver
unavailable(arrangements need to be made for a safe and appropriate
discharge setting such as SAF/SNF, home care,etc.)
• Complication of ambulatory surgery/procedure
• Extended observation post anesthesia/surgery
• Symptoms unresponsive to at least 4 hours Emergency Room
treatment.
• Psychiatric crisis intervention/stabilization with observation every 15
minutes.
• At 24 hours if the patient is not stable for discharge,apply acute care
criteria, extend observation up to 48 hours or refer for medical review.
• Then there is Interqual level of care designated as Critical Care for
CARDIAC,MEDICAL AND SURGICAL/TRAUMA.Where
SEVERITY OF ILLNESS(ONSET WITHIN 24 HOURS).Rule greater
or equal to one criteria.
• In acute Cardiac Event any of the following is required:
• Invasive hemodynamic monitoring
• Urgent temporary pacemaker insertion.Urgent Cardioversion.
• Balloon Pump(IABP)/left ventricular assist device LVAD.
• Acute intubation.
• Mechinical ventilation.
• Initial ventilator weaning.
• Nursing intervention at least every 1 to2 hours.
Post surgery/procedure requiring monitoring at least every 1 to 2
hours.
Severity of I llness onset within 24 hours .
• Clinical findings.
• ECG Findings.
• Laboratory Findings.
I n tensity of Service .( At least Daily) PO Medications
exclude unless noted.
• Acute uncomplicated MI care less or equal to 24 hours.
• Antiplatelet agents
• Balloon pump.(IABP)
• Blood more than or equal 3 units/24 hours and Hct less than
25%.
• Ca Gluconate
• Cardioversion urgent.
• Glucose 50% with insulin
• ACE inhibitors
• Antiarrhythmics.
• Antihypertension.Ca Channel blockers.Nitroglycerine
• KCL more or equal 10meq/h.
• Systolic BP Less than 90mmhg decrease from
baseline.Nitroprusside .
