R.N.-Case Manager

As a case manager I am in charge of the primary in-home or facility care of a patient under the supervision of the primary care physician. I set up medications for the patient when needed. I assess the patient each visit I make. I contact the primary physician when there is a need to do so. I keep the primary physician informed of any changes in condition of the patient. I draw blood for lab tests that may be requested by the physician. I do direct patient care when needed and assist the Home Health Aides when needed with direct care.

I have replaced peg tubes when they have needed to be changed. I have worked with ports, colostomys, urostomys, tracheostomys and all sorts of catheters.

I have worked with wounds and decubitis doing direct dressing changes and monitoring the care of them.

I have worked with the Care Anyware program since it was introduced to our organization most recently learning to enter the referrals on the live side of the program. I can readily admit patients and enter assessments on the CPOC side of the program.

I am a team player with my patients being my primary concern becuse I know that the patient and their family are depending on me to know my job well and to do that job to the best of my abilty.
