August, 1996 to present. Children s Rehabilitation Services, Flagstaff Medical Center, Flagstaff, AZ. Occupational Therapist for orthopedic and Neurology clinics, serving clients from Northern half of Arizona.

August, 1997 to May, 1999. Northern Arizona University, Institute for Human Development, Flagstaff, AZ. Instructor, NAU graduate level courses on Augmentative and Alternative Communication, and Assistive Technology.

October, 1998 to May, 1999. Northland Supportive Services, Ken Jacobs, PT. Provided occupational therapy to rural schools on contract

basis.

October, 1997 to Dec., 1998. Cottonwood-Oak Creek School District, Cottonwood, AZ. Self-employed contract therapist in school district. Lead in team for augmentative communication services for students.

September, 1996 to May, 1997. Self-employed, consultant occupational therapy to Kayenta Unified School District, Kayenta, AZ.

November, 1995 to present. Flagstaff Unified School District, Flagstaff, AZ. Substitute teacher, grades K-6, library, art and special education.

September, 1993 to September, 1995. OCM BOCES, Syracuse, NY. Provided OT services to rural public schools.

August, 1990 to May, 1993. Idaho School for Deaf and Blind, Gooding, ID. Developed OT program for residential state school serving visually and hearing impaired. Provided evaluations, direct service, and supervision of teacher's assistant for program implementation. Assisted adaptive P.E. in swimming instruction. Students served ranged from profoundly retarded to learning disabled, physically, visually and hearing impaired.

August, 1989 to May, 1990. Treasure Coast Therapists, Inc., Vero Beach, FL (Jean Burdsall). Provided contract therapy services to public schools.

January, 1986 to June, 1989. Indian River County School District, Vero Beach, FL. Teacher, intermediate grades. Grade level chairman for two years.

October, 1985 to August, 1986. Provide contract therapy services to public schools though Jean Burdsall, OTR, Vero Beach, FL.

May, 1983 to September 1985. Indian River Hand Rehabilitation, Vero Beach, FL. Staff therapist. Provided regular care and evaluation to hand therapy patients, developed numerous instructional aids, developed and implemented work adjustment service for clinic to prepare patient to return to work. Latter involved job task analysis, physical capacity evaluation and work hardening program.

November, 1979 to April, 1983. Greenleaf Hospital, Bryan, TX. Head, Activities Therapy. Developed and implemented Activities Therapy program for a new private psychiatric hospital, serving three units - general adult, adolescent, and chemical dependence. Supervised departmental staff, coordinated A.T. with other therapies, provided direct patient care, developed policies and procedures for A.T. (including manual), designed remodeling of areas for use by A.T., provided inservices.

April, 1978 to October, 1979. Self-employed, provided occupational therapy to rural public schools on contract basis.
