September 2, 2008-June 30, 2009: Pinehurst Surgical Clinic, 5 First Village Drive,
Pinehurst, NC 28374, Provided pre and post-op evaluations for major and minor
Vascular surgery patients. Daily office hours 8-6 or until the clinic and procedures were
completed. Reviewing and documenting labs, appropriate studies and specialty studies.
Conferring with my Primary Supervising physician regarding problem patients and
abnormal test results and scheduling patients for physician intervention.
September 2006-August 2008: Adult Nurse Practitioner at Moore Free Care Clinic, 705
Pinehurst Avenue, Carthage, N.C. 28327. Provided primary care to Indigent at risk
population of patients with cardiovascular disease, diabetes, anxiety, depression,
obesity, orthopedic problems, chronic pain, sleep disorders, menopausal/female issues

May 2005-August 2006: Wadesboro Family Medicine-provided primary care to
adolescents to geriatrics. Made rounds at Anson Community Hospital, Lillie Bennett
Nursing Home, and Sally Johson Nursing Home. Was responsible for writing daily
progress notes, updating orders, reviewing labs/xrays, referrals for appropriate consults,
took call every other week for our patients.
March 2005-October 2007-Anson Community Hospital-Telemetry Unit:Worked as RN
on 13 bed Telemetry unit providing direct patient care.
April 2003-January 2005-Sandhills Internal Medicine-provided primary care to
adolescents to geriatrics. Made rounds at two hospitals in Richmond County. Was
responsible for admission H & Ps, Discharge summaries, Nursing Home Orders, progress
notes, reviewing daily labs/xrays and specialty studies, consults as appropriate, taking
call every third night and week-end.
February 1998-December 2002-Hamlet Asthma and Allergy-provided primary care to
adults –geriatrics. Made rounds at two hospitals, updated pts progress notes, writing
orders, dictating H & Ps and Discharge summaries. I was initially hired to assist with
dictation and documentation on a PRN basis and then covered the office full-time while
Dr Shilen had hip replacement surgery.
July 1988-February 2002-Director of Nursing-Unifour Nursing Service-Rockingham,
N.C. I evaluated each patient for appropriate level of care required and developed the
Plan of Care and reviewed it with the assigned care giver and family. I was responsible
for delivery of and documentation of continuing education hours for all staff. I provided
annual updates for Bloodboorne Pathogens, Hepatitis, Aides, Universal Precautions,
Safety in the Home Settings, and Fire and Safety as required by the Department of
Facility Services. I met with the evaluation team from Raleigh at least bi-annually for
chart reviews and our office was never cited for any deficiencies in documentation or any
during their home evaluations.
January 2002-February 2003-First Choice Medical Care-provided primary care to
adolescents to geriatrics. Made rounds at two hospitals in Richmond County, dictated H
& Ps, Discharge summaries, wrote daily progress notes, reviewed daily labs/xrays and
wrote appropriate orders for each patient. Took call every other night and week-end, for
our patients, and unassigned call.
May 1994-December 2001-Hamlet Internal Medicine-provided primary care to
adolescent patients to geriatrics in Internal Medicine and Nephrology practice. Very busy
practice with a variety of very ill patients. Made rounds at two hospitals in Richmond
County, writing appropriate orders, dictating admission H & Ps, Discharge summaries,
progress notes, Nursing Home Orders, reviewing/updating No Code Orders, Restraint
Orders, Dialysis orders. Made rounds at Britthaven Nursing Home, Country Manor
Nursing Home and Palmer Hinson Nursing Facility. I wrote monthly summaries,
updated necessary lab orders, documented exceptional events(UTI, ER visit, etc), and
talked with families.
1991-1994-ICU-ER-Baylor week-ends-Charge Nurse/cross trained both areas. Primary
care to patients in both units as assigned. Also served as House Supervisor every 4th
week-end, at Richmond Memorial Hospital.
1989-1991-Staff Development Director-Richmond Memorial Hospital. Coordinated the
in-services for all the hospital staff, kept detailed records of continuing education hours
and required programs for all departments. Taught BCLS classes to hospital staff and
community participants. Developed and taught the CNA I class and taught the first two at
RMH. Taught numerous classes to the general public on a variety of topics.
1987-1990-Staff Nurse PACU-provided direct patient care for patients reacting from all
types of anesthesia;general, spinal, caudal, saddle block, regional block, local, topical,
epidurals, and/or any combinations of the above. Functioned as Chargenurse in the
absence of the Nurse manager. Assisted physicians with the insertion and calibration of
arterial lines, Swan-Ganz catheters, CVP lines, ventriculostomies, chest tubes, Tenekoff
catheters and ventricular drains. Maintained and reassessed the patients airway airway
and initiated emergency protocols when required. Correctly calculated Heparin, Isuprel,
Dobutumine, Dopamine, Pitocin, Duromorph, Lidocaine and PCA drips.
1988-1990-Staff Nurse Float Pool-Marlboro Park Hospital, Bennettsville, SC. Worked
any unit in the hospital, including L & D. Usually worked 16 hours in ICU.
February 1988-May 1988-Director of Nursing-Country Villa Nursing Home, Hillhaven
Corporation, Rockingham, NC. I was in charge of the day to day needs, staffing, problem
solving for the entire staff in the building except maintenance. I interviewed, screened,
hired, counseled, and terminated the staff accordingly. Iwas responsible for the QA and
QI monitoring and data reporting to the company headquarters. I was present for DFS
reviews and and there were no Category 2 citations and only 3 Level 1s which were
corrected and the written Plan of Action received by DFS in 30 days and f/u review was
100%.
September 1987-February 1988-Rehabilitation Coordinator for American Rehab. Direct
management for workmens compensation, liability and long term disability cases
covering a large territory from Charlotte to Goldsboro. Planned, monitored and evaluated
treatment for all assigned patients. Provided written reports daily/weekly as conditions
warranted.
December 1986-June 1989-ICU-Staff nurse 7p7a providing primary care to assigned
patients in ICU. We had a heavy cardiac patient population but occasionally received
trauma, orthopedics, surgery or peds cases.
August 1986-1988-Staff nurse Float Pool Moore Regional Hospital-I worked whatever
unit I was needed but Waldrip CCU was short staffed and I spent a lot of time between
there and Neuro ICU.
March 1984-August 1986-Head Nurse ICU-Chesterfield General Hospital, Cheraw, SC.
Head Nurse ICU, Week-end supervisor, Quality Control Coordinator. As Head nurse I
was responsible fo all the daily activities of my unit. I did the interviewing and hiring.
I did the QA/QC monitoring, budget, staff evaluations, counseling, and or termination if
needed. I acted as weekend supervisor and took administrative call for the hospital on a
as needed basis. I responded to all Codes, assisted in the delivery room and/or wherever
necessary.
February 1983-March 1984-IV Team Manager-We were contract services to the hospital
in SC to provide IV therapy to their patients, in-service to the staff and assist the
physicians as needed.
March 1982-February 1983-Staff nurse Open Heart Unit Mercy Hospital, Charlotte, NC.
I provided direct patient care to cardiovascular/thoracic patients pre and post-op-critical-
