Responsibilities
The LVN did daily monitoring of signs and symptoms of psychological and physical medications
with side effects exhibited by patient. Skin assessments done daily along with glucose and blood
pressure monitoring when requested by physician. Transfer of patients to commode or bed when
ordered by physician. Quarterly progress faxed to physician. Reassessment and medical update
every six months faxed to physician for evaluation and, sent back to LVN for entry into data base
for continuing medical at the center. All data entry and updates were done by the LVN. Transport
of patient to the E.R. for assessment by the physician when medically concerned about patients
change of condition. . Phone calls to pharmacies and physician offices when necessary.
Medication updates are data entered when notified of new or discontinued medications. Daily
and monthly flow sheets filled out along with monthly summaries on all. Patients monitored
closely with meals for choking or patients roaming about. Diaper changes when necessary with
skin assessment. Pain scale implemented with patient when monitoring for signs and symptoms
of discomfort or pain. Treatments include hot packs or Tylenol, when necessary. Meals are
observed with special consideration for choking. The LVN accompanied on field trips with
patients for monitoring of concerns from list above. Medication set up on a weekly basis for
psych patients.

My position as LVN in sub acute is very challenging. I work with ventilator patients whose
condition can change from minute to minute. They are closely monitored by the LVN for this
reason.
The duties of the LVN in sub acute are: Treatments and medications administration by LVN.
Close monitoring for change of condition in regards to medications and treatments administered.
Monitor for signs or symptoms of shortness of breath. Tracheostomy dressings and care on
every shift. NG, JT, NGT tubing maintained every shift with orders as to the proper managing of
patent tubing and feedings that need to be administered through this tubing. Skin assessed in
regards to this type of feeding around tubing and in general for basic skin assessment integrity. I
monitored signs and symptoms of restlessness, anxiety, pain, depression, or, shortness of breath.
Documentation on all treatments, medications administered, or, any occurrence that is unusual
not mentioned above. Check oxygen as to correct setting along with tube feedings and correct
nutrition being administered. Report to charge nurse any unusual/abnormal occurrence from
patient. The LVN suctioned patients according to orders and, as needed by patient.
