Family Case Manager
Duties: In-take and management of all temporary residents transitioning from
homelessness (substance abuse) into our program. Responsible for meeting with
and supervising residents on a weekly basis, as well as implementing a case plan
and future goals, helping resident set up and maintain a monthly savings and
budget, collecting required monthly program service fees, generating referrals
for childcare, therapy, transportation and homeless services assistance.
Monitoring residents via daily home visits, conducting monthly resident-family
meetings, organizing activities for families (birthdays, holidays, outings and
special events), and documenting services provided to all residents in our

safety plans, status reports, and home studies. Preparing for court
hearings, judicial reviews, dispositions, mediations, arraignments, permanency
reviews and reports of progress and safety for the child and his family.
