assigned staff in a community outpatient mental health center and in an involuntary
inpatient psychiatric unit.

Group Health Cooperative – Hospice/Homecare Social Worker: 09/2001 through 06/2011.
Primary Responsibilities: Provided advanced-illness and end-of-life consultations to Group
Health members in Providence Colby Campus in Everett and Overlake Hospital. Conducted
comprehensive biopsychosocial assessments, diagnoses, treatment planning, evaluating
effectiveness of interventions. Enacted empirically substantiated therapeutic interventions for
anxiety, depression, chronic and complex biopsychosocial conditions. Engaged in family
counseling specific to chronic medical advanced illness, mental health, grief/loss, and end of life
counseling. Completed required charting and other administrative duties as identified in state
and federal requirements.
Harborview Medical Center -AIDS and Mental Health Social Worker: 07/1995 through 09/2001.
Primary Responsibilities: Conducted comprehensive mental health and biopsychosocial
assessments with youth and adults newly diagnosed with HIV/AIDS to end-stage. Provided
outreach and ongoing monitoring services to the individuals and families affected by HIV/AIDS,
chronically mentally-ill, chronically chemically dependent, homelessness, and chronic
incarceration. Delivered specific motivational interviewing techniques to individuals with
HIV/AIDS and practicing unsafe sex practice - as part of a King County Health Department/
University of Washington/Harborview study. Co-chaired Harborview clinic safety committee.
Co-created current emergency domestic violence assessment form used in Harborview
Emergency Department.
Overlake Hospital Medical Center – Psychiatric Social Worker : 07/92 through 07/1995.
Primary Responsibilities: Conduct psychiatric inpatient mental health assessments. Participate in
interdisciplinary team meetings in which individualized treatment plans including discharge
plans were delineated. Conducted family education groups. Cross-trained in providing social
work services at Overlake Emergency Department.
Eastside Mental Health, Inc. – Team Lead and Supervisor: 06/1990 through 07/92.
