Provide case management for a patient load of 13 -18 patients. This included twice weekly home and group home visits with physical and situational assessments. Practiced infection control within the home and group home environment while assisting family and caregivers in their infection control practice. Educating family and caregivers on how to best provide comfort for the patient in the death process, to include education regarding expected decline and death process. Following Medicare guidelines to provide continued hospice services and document continued decline. Obtaining resources to including inpatient respite, volunteer visits and DME to assist the family in caring for the patient.
Feb. 2003 – Dec. 2009 Mayo Clinic Hospital, Phoenix, AZ. Liver Transplant Coordinator.
Provided hospital discharge teaching for the fresh transplant recipient and their caregiver. Staffed post-transplant clinic twice weekly to include assessment of learning and to provide care for urgent issues as determined by the physician. Phone triage and follow up, for both pre- and post-transplant patients. Insurance prior authorization obtained for testing, immunosuppressant medications and follow up testing for ongoing medical needs of the transplant recipient. Planned necessary follow up and testing to accommodate out of town patients with the need for a compact schedule. Lab review for all post-transplant patients and transmission of verbal orders given by MD for medication changes etc. to patients. Education for post-transplant patients who initiated HCV treatment to including how to dose and inject necessary HCV medications and growth factors as needed.

Worked registry in anticipation of the move to Phoenix. I needed flexibility in hours and self scheduling. I functioned as both a staff nurse and a charge nurse at a number of local ERs.
May 1990 – Nov. 1998 Parkland Memorial Hospital, Dallas, TX.
Worked as a staff nurse, charge nurse and preceptor for new graduates in a very large and busy county hospital ER, we typically saw 300 -500 patients a day. It was a Level 1 trauma Center and the burn center for Dallas and Ft. Worth. I worked as a triage nurse typically triaging at least 150 patients in a 12 hour shift. My personal record was triage and full assessment of 172 patients in a full 12 hour shift. During my tenure I was a preceptor for over 30 new graduates several of whom became unit managers. Also worked in OB/GYN ER and psychiatric ER duties included chaperone of rape exams and collection of evidence to aid in prosecution of the perpetrator. Assisting police and doctors in obtaining mental illness warrants, collection of evidence to be used in court for prosecution of DUI’s and murder investigations. Maintaining chain of custody for both the ME and police to preserve the integrity of all evidence collected.
