• Family Case Manager – Identified and enrolled eligible pregnant women through telephone contacts, home visits, community canvassing and referrals from public agencies.

• Assessed participants’ risk factors and developed service plan to meet participant's needs.

• Provided support to over thirty female participants and their families by establishing priorities and following a plan.

• Linked participant to appropriate preventive health and social services as needed and provides follow-up on services throughout the prenatal and post-natal period.
