Assistant Program Director of Brooklyn Day Habilitation 1 and 3

9/2011 to 6/21/13 UCP of NYC, Brooklyn, NY

Oversee the day to day operation of 2 Day Habilitation program consisting of approximately 168 consumers, 12 Habilitation specialist as well as 48 Habilitation Assistant, 2 Program Coordinators and a 6 of clinicians.

Directly oversee the agency ADP system of staff clocking in and out. Providing approvals on a weekly basis

Review and approve Individual Service Plans for 96 individuals

Initiate and analyze goals implemented on a semi-annual and annual.

Review and authorize monthly report from Habilitation Specialist

Communicate directly and indirectly to our families as needed.

Provide counseling to address behavior with in the Day Program.

Review Behavior Management Support Plan, train Habilitation Specialist on plan annually and or if an amendment was made.

Provide counseling to individuals who are going through an adjustment stage, whether adjusting to a new environment (Program) and/or changes within the class.

Participate on the agency borough committee, to implement changes as needed.

Sit on the agency Safety Committee- To determine areas of the agency current Safety Policy which may need changes or adjustments.

Process internal investigations as needed

Participate on monthly internal audit team through the 5 boroughs of New York City

Interview new applicant and work directly with the Human Resource department until staff is hired

Schedule staff training under the rules and regulations of OPWDD on a 6 months basis

Complete monthly walk through with the Building Services Department to ensure that the building is up to OPWDD code.

Residential Program Director of Respite

2/2008- 9/2011 UCP of NYC, Bronx, NY

Oversee the day to day operations, 24 hours 7 days per week

Provide direction to a team of 16 Resident Program Specialist, 1 Administrative Assistant, 1 Nurse, 1 Resident Manager

Provide direct oversight for the intake process which was on Mondays and Fridays with a maximum of 10 consumers on each day,

Provide direct communication with families of which we served. UCP of NYC Respite program serve all 5 boroughs of NYC.

Ensure transportation was implemented.

Process monthly billing on the OPWDD website, as well as process monthly billing to individual borough OPWDD sites.

Oversee staffing which include but not limited to weekly scheduling, days off request and FMLA process when needed.

Oversee training schedule as well as provide training to ensure OPWDD standards are being met.

Provide counseling to individuals to address behavior within the facility.
