Shift: Day, 5x8-Hour (08:00 – 17:00), Monday–Friday
Mileage: Reimbursement provided at federal rate
Travel Requirement: Must have own vehicle; daily travel required throughout the community
Experience: Prior travel nursing experience required
System Requirement: HCHB (HomeCare HomeBase) experience required
Responsibilities
Manage a caseload of hospice patients, providing compassionate, patient-centered care in the home setting.
Conduct comprehensive case management, including assessments, care planning, and coordination with interdisciplinary teams.
Deliver skilled nursing interventions such as symptom management, medication administration, and patient/family education.
Document care accurately and efficiently using HCHB.
Collaborate with physicians, social workers, and other care providers to ensure continuity of care.
Minimum Qualifications
Active RN license in North Carolina (or compact state).
Minimum of 1 year case management experience.
Proficiency with HCHB charting system.
Strong communication and organizational skills.
Ability to work independently and manage daily travel responsibilities.
