Key Responsibilities
Lead and ensure full utilization of the interdisciplinary team during the formulation and completion of all MDS/RAI assessments.
Conduct thorough observations of direct nursing care, review clinical documentation, and make timely, appropriate recommendations to improve care coordination.
Assist in performing chart audits, ensuring accuracy, compliance, and alignment with state and federal regulations.
Provide ongoing clinical support, guidance, and education to the care team using appropriate resource materials.
Maintain strong working knowledge of MDS 3.0 processes, scheduling, and submission timelines.
Ensure compliance with Medicaid and Medicare regulations related to RAI processes.
Collaborate with department leaders to support care planning, reimbursement processes, and quality improvement initiatives.
Promote Grant Healthcare & Rehabilitation’s dedication to high-quality, resident-first care with professionalism and empathy.
Requirements:
Current license as LPN/RN in State.
Certification in MDS
Previous MDS experience gained in Skilled Nursing Facilities
Must know MDS 3.0 Experience
Knowledge of Medicaid and Medicare
Must have long-term care experience
Excellent attention to detail, communication skills, and ability to collaborate across departments.
