What Youll Do as a Registered Nurse Case Manager
As a Registered Nurse Case Manager, youll be the cornerstone of our care teamhelping patients live their best quality of life. Each day youll:
Coordinate and manage individualized hospice plans of care
Deliver hands-on nursing care and emotional support to patients and families
Educate caregivers and guide them through the hospice journey
Collaborate with physicians, social workers, and chaplains to ensure holistic care
Respond promptly to changes in patient needs with expertise and compassion
What to Expect
This full-time RN Case Manager role is 40 hours per week, MondayFriday, with a rotating on-call schedule for evenings and weekends. Youll work independently while always supported by a caring team.
What Youll Bring
Current Illinois Registered Nurse (RN) license
Graduate of an accredited nursing program (BSN preferred)
At least 1 year of medical/surgical, oncology, or hospice nursing experience (2+ years preferred)
CHPN certification preferred (or obtained within 2 years)
CPR certification (within 3 months of hire)
Strong clinical judgment and ability to work independently
Local residence with ability to respond to on-call within 30 minutes
