What You'll Do: ️ Assist families with basic needs and resources to promote self-sufficiency ️ Develop service plans with measurable goals ️ Provide in-home services when appropriate ️ Visit each client at least once per week ️ Facilitate networking and linkages with other community services ️ Maintain accurate case notes and records ️ Complete referral forms, social histories, and closure reports within designated timeframes ️ Participate in team meetings and staff reviews to evaluate client progress ️ Follow up on referrals and coordinate necessary services
What We're Looking For: Education: 2 years of college at an accredited institution (Associate's Degree or higher preferred) Experience: Knowledge of trauma-informed care and willingness to engage in ongoing training Requirements:
Valid driver's license, reliable vehicle, and state-required car insurance
Ability to pass State and Federal background checks
Strong communication and organizational skills
Ability to work independently and within a team
Comfortable providing home-based services to families
