GENERAL STATEMENT OF DUTIES
Elbert County Public Health Department is seeking a compassionate, dedicated, and highly skilled Nurse Practitioner (NP) to provide primary and preventive health care services to individuals and families across their lifespan. The NP will function as an integral part of a multidisciplinary team to deliver high-quality care in a public health setting, with a strong focus on underserved and vulnerable populations.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Conduct comprehensive health assessments including physical exams, medical histories, and screenings
Diagnose, treat, and manage acute and chronic health conditions
Order, perform, and interpret diagnostic tests and lab results
Prescribe medications and non-pharmacologic treatments in accordance with state laws and collaborative agreements
Provide patient education on disease prevention, healthy lifestyle choices, and chronic disease management
Collaborate with physicians, nurses, case managers, social workers, and public health staff
Participate in health promotion and disease prevention initiatives
Maintain accurate and up-to-date electronic health records (EHR)
Provide oversight and assist in developing clinic and public health policies and procedures
Adhere to clinical protocols, policies, and standards of care
Participate in quality improvement, data reporting, and public health research initiatives as needed
Support emergency response efforts and community outreach activities
Maintain compliance with all HIPAA and confidentiality requirements
Completes job specific training as required
Various additional duties as assigned
KNOWLEDGE AND SKILLS
Strong clinical judgment and diagnostic skills
Excellent communication and interpersonal abilities

Ability to work both independently and collaboratively
Proficient in electronic health record systems and clinical documentation
Commitment to public health principles and community wellness
Basic medical coding knowledge is preferred
EDUCATION, EXPERIENCE AND CERTIFICATIONS
Master’s Degree in Nursing (MSN) from an accredited program
Current, unrestricted Nurse Practitioner License in Colorado
Certification as a Family Nurse Practitioner (FNP)
Basic Life Support (BLS) certification
Current DEA license and prescriptive authority (or ability to obtain)
Department of Transportation (DOT) Certification - preferred
A minimum of 1-2 years supervised experience as a nurse practitioner is required
ADDITIONAL REQUIREMENTS
Valid Colorado Driver’s License
Acceptable Motor Vehicle Record
Must be able to pass a criminal background check and drug test
Bilingual a plus
