OBJECTIVE:
The Supported Living Coordinator is responsible for the coordination and delivery of homemaker personal care services for intellectually and/or developmentally disabled adults in a supported living environment. The Supported Living Coordinator plans, organizes, directs, coordinates and manages supports provided by the Direct Support Professionals and the Lead DSPs. The SLC works to enhance the attainment of the Individuals’ personal goals and objectives while maintaining required standards of the various regulatory agencies including ensuring health and safety of the Individuals.
PRIMARY RESPONSIBILITIES: (including, but not limited to)
• Supervises Direct Support Professionals and Lead DSPs – Advising H.R. as to performance for reviews, feedback, and corrective action if needed.
• Schedules and conducts monthly Lead DSP meetings.
• Attends/Schedules/Coordinates meetings for Behavior Support, OISPs, Team meetings and Staff meetings as needed.
• Monthly reviews of all site documentation including but not limited to: OISP service delivery documentation, financial documentation, medical appointment documentation and tracking, MAR’s, shift log notes, behavior data and forms ensuring health and safety.
• Completes monthly site visits and maintains communication with members of the Individual’s team at least monthly.
• Completes QA reviews monthly.
• Communicates with Adult Day Service Providers monthly.
• Reviews of Fire/Safety checklists and faxed to the FCBDD SSA within 5 days of completion.
• Follows all guidelines for UI/MUI reporting per the Ohio Revised Code.
• Works open shifts as needed.
• Completes LPS billing monthly and turns in by the 5th of each month.
• Provides back up support to the Lead Staff on-call
• Assists with planning and coordination of outings, social events, and holiday events.
• Assists with scheduling and ensuring Individuals are receiving services as outlined in the OISP.
• Monitors and reduces Over Time hours.
• Enters approved PTO and Time Off Requests (Including Floating Holidays).
• Reviews EVV entries to ensure accuracy of billing by 10:00am on Mondays each week.
• On-Call as required.
• Checks voice mail and e-mails daily, and responds within 24 hours.
• Assists with training and orientation as needed.
QUALIFICATIONS:
• Bachelor’s Degree or equivalent experience required.
• Must be able to work effectively for and with individuals who have Developmental Disabilities
• A minimum of five years of experience in the ID/DD industry or related field
• A minimum of three years supervisory experience
• Ability to pass a background check and drug test
• Must have own, reliable transportation, a good driving record with no more than 5 points on license, a valid driver's license and insurance meeting State of Ohio liability requirements.
• Medication Administration Training, CPR & First Aid
• 8 hour DODD Training and Additional training as needed or required
• Ability to communicate clearly both verbally and in writing.
• Ability to multi-task, meet deadlines and work autonomously.
• Ability to maintain clear boundaries with individuals and employees.
• Demonstrated sound judgement and self-direction.
• Must be able to work with interdisciplinary teams
• Knowledge of Microsoft Word, Excel, Publisher, Power Point
• Knowledge and experience with Advisor Anywhere preferred
• Must present a professional appearance and demeanor
