Central Ohio Primary Care is seeking a full-time Advanced Practice Provider (NP/PA-C) for a Family Medicine practice in Columbus.
The APP, in collaboration with the Physicians, coordinates the healthcare of primary care patients at this location. This position monitors the patient’s status and collects data to evaluate and improve care by providing a comprehensive assessment, implementing the appropriate treatment, and evaluating the patient’s progress.
About the practice:
The work schedule would be 5 days a week, M-F.
Key Responsibilities:
Conduct thorough assessments, including medical history reviews and physical examinations, to diagnose and treat acute and chronic health conditions in patients of all ages.
Provide a full spectrum of primary care services, including well visits, health promotion, disease prevention, routine screenings, vaccinations, and management of common illnesses and injuries.
Develop individualized treatment plans for patients with chronic diseases such as diabetes, hypertension, asthma, and COPD. Monitor disease progression, adjust treatment regimens, medication management and educate patients on self-management strategies.
Empower patients and families by providing comprehensive education on health maintenance, disease prevention, and treatment options. Address questions, concerns, and health-related inquiries to promote informed decision-making and adherence to care plans.
Collaborate with physicians, advanced practice providers, nurses to coordinate patient care across multiple disciplines and healthcare settings. Facilitate referrals to specialists and other healthcare providers as needed.
Maintain accurate and timely documentation of patient encounters, treatment plans, and follow-up care using electronic health record systems in compliance with regulatory standards and organizational policies. COPC uses EPIC.
Qualifications:
Master’s degree in nursing or physician studies from an accredited program.
Current licensure as a Nurse Practitioner or Physician Assistant in state of Ohio.
National certification as a Family Nurse Practitioner (FNP) or Physician Assistant (PA).
A minimum of 1 year of practical experience as a NP/PA in a primary care outpatient practice setting.
Previous experience managing a patient panel in an outpatient setting is preferred.
Strong clinical assessment, diagnostic, and treatment skills.
Excellent communication and interpersonal skills.
Ability to work independently and collaboratively within a multidisciplinary team.
EPIC EHR experience is needed. Training is available at onboarding.
