Shift: 8hr Day Shift (40)

Shift: 8:00 AM to 4:30 PM; Monday through Friday

On Call, Call Back Requirements: none

Weekend Rotation: none

Holiday Requirements: none

Floating Requirements: Within Service Area- possible to float to neighboring areas

Patient Service Area (PSA) DNU: Continuing Health PSA

Coverage area: 53097, 53209, 53217, 53218, 53223, 53224

Candidate MUST HAVE:

- 2 years PT Home Health experience with Adult/Geri patients.

- active PT license (WI) and BLS (AHA)

- valid Driver's License and auto insurance
