We are a small, but growing, private pay home care agency located on the shoreline of Connecticut looking for a dynamic individual to serve as a Program Manager.

The Program Manager promotes and provides high-level coordination of the home care services and oversees the Care Management team. Must be customer-service orientated and maintain relationships with clients and referral partners. Leadership experience with private pay home health organizations is a plus.


•Healthcare / Private-Duty home health leadership experience preferred.

•Private Pay / Long Term Care Insurance payor experience.

•Strong customer service and relationship management skills

•Ability to work with an all-inclusive, and diverse clientele.


•Effectively cultivates new business opportunities.

•Initiate and maintain recruiting efforts.

•Oversee patient / client care team members.

•Regularly communicate with referring partners, clients and their families.

•Coordinate care and create care plans for incoming clients

•Maintain client documentation, records, and monitor metrics.
