2 Work Schedule options:

Day: 7am – 7pm; Night: 7pm-7am; 3 days, 12hrs/day, Every other weekend.

Pay: $20-25/hr.


The LPN is responsible for the support of the RN staff at our assisted living homes for delegated clinical tasks and other assigned duties.


Familiar with current resident service plans

Monitor residents for changes in condition

Receive and perform delegated tasks given by RN

Prepare physician rounding forms before visits

Prepare and confirm resident forms for auxiliary services

Facilitate dentist and podiatrist visits

Work with hospice companies to receive Certified Hospice Assistant

Per RN delegation maintain resident finger nails and toe nails

Per RN delegation administer injections

Per RN delegation assist with wound dressings for residents

Monitor PRN medication stock for residents and submit reorders

Travel to pharmacy when medication is needed to be picked up

Monitor pass down and confirm narcotic counts

Monitor forms at homes to ensure needed copies are available

Monitor daily medication packs

Review incident reports with RN

Prepare medication administration training packets

Maintain clinical supply inventory and submit request

Communicates with families, responsible parties, case managers

May participate in the orientation of new staff

Communicate to RNs any issues with employee performance, problems

Participates in care conferences with other members of the health care team when required

Communicate to RNs, observed training needs of Care Partners

Promotes good safety practices:

-Body mechanics

-Reporting of defective equipment or hazards

-Wiping spills or wet floors immediately

-Using Universal Precautions and appropriate infection control

-Knowledge of emergency procedures

-Reporting incidents to appropriate family member/s

-Reporting incidents/accidents to the Administrator

Promotes a safe environment for the residents and family

Participates in in-service educational programs

Adheres to all MN Department of Human Services rules

Has completed the required training for Alzheimer’s and Parkinson’s



-We consider the safety of our residents a priority.

-We improve our service continuously through education.

-Graduate of accredited LPN program and has a current LPN license

-Experience in home care, long term care, or assisted living

-Supervisory experience preferred

-Able to function independently

-Has a current, negative TB test
