Position Available: Clinical Case Manager (Full Time)

Supervisor: Program Director, The Fellowship Initiative/Assistant Vice President, Center for Policy, Advocacy and Education

Position Summary: Vibrant Emotional Health is seeking two exceptional candidates to provide clinical case management services for The Fellowship Initiative Program (TFI) program, a college-attainment program for young men of color across four cities, including New York, NY, Chicago, IL, Los Angeles, CA and Dallas, TX. The candidates we are seeking must reside in the Los Angeles, CA or Dallas, TX areas.

This is a full-time position with responsibilities focused on providing clinical case management services to TFI Fellows at several program sites around the country. The positions will be based in Los Angeles and Dallas only and will report to the Program Director, who is based in New York City. Supervision will be provided remotely, with occasional travel to New York City.

Essential Duties and Responsibilities:

•The position will be based in LA and Dallas, with occasional travel to both Chicago and New York TFI locations, at least two Saturday's per month.

•Conduct yearly emotional wellness assessments of “Fellows” (youth in program, ages 16-18) to identify social-emotional needs.

•Provide on-site training's and workshops for TFI parents, staff and mentors.

•Facilitate social skills curriculum for youth.

•Develop measures to assess efficacy of emotional wellness services (yoga, mindfulness).

•Engage with alumni/graduates of TFI program and refer to services in their college communities.

•Assist Program Director in identifying community resources and mental healthcare providers.

•Match Fellows with local providers.

•Establish payment agreements between Vibrant and providers.

•Service coordination when Fellows are involved with multiple providers or systems (legal, foster care, psychiatry, etc.)

•Maintain records of all case contacts.

•Participate in bi-monthly supervision with Program Director.

•Consult with Clinical Consultants when appropriate.

Qualities:

•Passionate about working with youth and families.

•Knowledge of adolescent social/emotional development, including clinical presentation of mental health challenges.

•Strong clinical assessment and engagement skills.

•Strong communication, presentation and group facilitation skills.

•Ability to remain calm in the face of youth and family crisis.

•Team player with willingness to “jump in” when needed.

•Ability to work independently and take initiative to develop projects.

Qualifications:

•Master’s degree in relevant field plus 2-3 years of experience in clinical case work.

•Experience working with diverse populations individually and in groups.

•Strong written and verbal communication skills.

•Crisis management skills.

•Proficiency in Microsoft Office Products (Word, Excel, PowerPoint, Access).

•Bilingual/fluent in Spanish strongly preferred.

•Ability to travel/work on weekends REQUIRED.
