• Position Summary: Works under the supervision of the RN Case Manager (CM) &/or Social Worker (SW) to assist with the coordination and implementation of the discharge plan of care. The position helps coordinate this complex process of efficiently and effectively moving the patient through the continuum of care to ensure timely discharge to the next appropriate level of care. The Discharge Specialty promotes the integration through effective and timely communication with the CM, SW, other multi-disciplinary team members, care providers, patients, families and external vendors.

• Principal Accountabilities:

1. Works independently to coordinate and implement the discharge plan of care developed by the nurse care manager.

• Works with facilities and vendors to obtain services/placement acceptance.

• Coordinates benefits and patient preferences with the patient/ family care team, insurance companies and community-based support services according to the discharge plan

• Coordinates effective linkages with community resources and alternate services for clients with limited benefits or personal resources

• Facilitates the transfer of a patient to an appropriate lower level of care such as Skilled Nursing or home with Home Health

• In collaboration with the discharging physician assure completion of Discharge Diagnosis Documentation paperwork

2. Monitors the plan of care and coordinates the delivery of services

• Identifies potential problems and brings them to the attention of the CM &/or the MSW to prevent and/or resolves variances to the discharge plan of care

• Collaborates with the CM/MSW and/or client/family to resolve conflicts regarding the treatment plan.

• Collaborates with CM/MSW to expedite timely resolution of situation such as client concerns or need for referrals to expedite the discharge plan

3. Documents information clearly and accurately to promote communication with patients, caregivers, vendors and customers

• Maintains accurate and current client records according to department standards.

• Documents in patient electronic record discharge disposition, DME and Transportation arrangements

• Obtains patient, family signature on 2nd Medicare Important Message letter

• Identifies and refers quality and risk management concerns to supervisor Complies with confidentiality policies, HIPPA regulations and department standards when transmitting patient information to affiliates, agencies or other vendors as needed for patient placements and referral

• Provides clerical supporting as needed including copying, faxing and data entry.

• Completes forms required for department reporting and analysis

4. Actively participates as case management team member to achieve department goals and objectives

• Attends regularly and actively participates in daily team huddles and Case Management department meetings.

• Positively contributes to team decision making process in planning daily priorities, resolving barriers to discharge plans and seeking creative solutions to resolve conflicts

• Collaborates with team members. Demonstrates both initiate and flexibility working with Physicians, Case managers, Assistants and other staff.

• Cross trains provides coverage to other department functions as required by supervisor

5. Promote customer service with internal and external customers

• Demonstrates excellent written and verbal communication skills

• Relates with tact and respect to internal and external customers with diverse cultural and socioeconomic background who may be exhibiting varying levels of distress

• Actively builds positive relationships with internal and external customers, especially affiliates and external vendors

• Uses effective communication skills with colleagues to resolve issues in a timely, positive and productive manner.


• Direct patient care or health education in an inpatient, skilled nursing, home health or related setting - Highly desired

• 1 Years’ Experience coordinating benefits and reviewing alternative with patients and families - Required

• General knowledge of durable medical equipment (DME) - Required

• Hospital experience - Required

• Working knowledge of medical terminology - Required


• A.A. Degree in Business/Health Related field - Required

• Bachelor's degree - Highly desired
