We have a new employment opportunity for a Social Services Assistant to provide Social Services assistance for a skilled nursing facility located in Concord, California.

The Social Services Assistant plays a critical role within the interdisciplinary team, orchestrating the coordination of multiple services to ensure that the psychosocial and long term medical needs of each resident are met.

Social Services Assistant responsibilities:

• Providing residents with comfort, safety, and security

• Ensuring access to enjoyable and meaningful activities

• Preservation of optimal level of function

• Maintaining resident individuality, privacy, autonomy, and dignity

• Nurturing spiritual well-being

• Providing access to available local resources

• Transport Assistance

Social Services Assistant qualifications:

• Nursing home social worker experience preferred
