Home Health Clinical Coordinator (RN) – Bellevue, WA

We have an exciting new employment opportunity for a Home Health Clinical Coordinator in Bellevue, Washington. The Clinical Coordinator facilitates compliance with all regulatory requirements. The Clinical Coordinator plays an active role in educating staff relative to care, service, and compliance issues. The position reports to the Clinical Manager.

Strong clinical skills are required. We require a Registered Nurse (RN) for this position.

Home Health Clinical Coordinator responsibilities:

• Shall be available at all times during the operating hours of the agency and after hours as needed for back up.

• Participates in activities relevant to the professional services provided including the development of qualifications and assignments of personnel.

• Conducts in-service education programs for field staff.

• Interviews, hires, and conducts orientation for all new clinical personnel.

• Assures compliance with all local, state, and federal laws regarding licensure and certification of staff.

• Ensures that all licensed and/or client care is physician directed.

• Ensures that all physician orders are current and being followed by agency personnel.

• Directs all home healthcare services provided by the agency by performing the initial assessment of client, determining the client care plans and appropriate level(s) of service, supervising the implementation, coordination, maintenance, evaluation, and termination of care plans and service(s).

• Supervises all agency services through the performance of scheduled and non-scheduled on-site client visits.

• Participates in personnel evaluations.

• Ensures proper maintenance of medical records as mandated by local, state, and federal law.

• Maintains a good standing in the medical community

• Monitors changes in home healthcare nursing and reports changes to Executive Director

• Develops policies and quality assurance program

• Assumes on-call responsibilities as necessary and directed by the Executive Director

• Performs other duties as assigned by the Executive Director

Home Health Clinical Coordinator qualifications:

• Must have a Bachelor of Science in nursing degree and have a valid nursing license in the state in which he or she is working.

• Must possess excellent administrative, management and communication skills, mature judgment, and emotional stability.

• Two years of experience in home care is preferred.
