A Leading a federally-qualified community health center that has five locations throughout the Chicago area and has more than 230 employees is seeking an experienced psychiatric nurse practitioner to join their team

The Psychiatric Nurse Practitioner will conduct diagnostic evaluations and prescribe appropriate treatments while utilizing evidence-based research within the scope of standards of practice for Advanced Practice Nurses as deemed by the American Nurses' Association. In collaboration with the health care team the Psychiatric Nurse Practitioner will work within the collaborative practice and assure smooth functioning of the medical team in the clinical setting by actively supporting Performance Improvement activities.

The Psychiatric Nurse Practitioner provides psychiatric care within an integrated primary care-behavioral health model, wherein the Psychiatric Nurse Practitioner actively collaborates with treating primary care providers and behavioral health providers, as well as care coordination team.

Provides direct patient care as a psychiatric nurse practitioner in the office, or other such locations within the community as assigned. This may include evenings and weekends. Engages patients in integrated treatment planning with all providers (medical, dental, behavioral health) and ancillary staff working with a patient. Consults with medical providers and behavioral health providers regarding recommended psychopharmacological treatment strategies to address patient need.

Participate in programs or task forces to meet the medical needs of patients. Chairs or attends meetings as assigned. Participates in the Quality Improvement and Peer Review programs. Manages the care of patients in collaboration with other clinical staff and members of the care team to make medical decisions on treatment and attain health goals.
