Hospice Intake Coordinator – Santa Monica, California

We have a new employment opportunity for a Hospice Intake Coordinator (LVN / LPN) who will provide holistic care to terminally ill patients by performing initial assessments and designing & implementing the plan of care in Santa Monica, CA.

Compensation will be based upon professional experience. This is a full-time position. The successful candidate will be a Licensed Nurse with a solid clinical background.

Hospice Intake Coordinator Responsibilities:

• Assessing the needs of terminally ill patients & families, addressing symptoms and providing comfort for the dying patient

• Developing and implementing a hospice care plan that includes medication, procedures, and treatments, while respecting the patient’s goals and family values

• Representing the hospice to the public, while promoting optimal customer service at all times.

• Receiving and documenting referrals promptly.

• Demonstrates commitment to company’s mission, values and standards of ethical behavior, professional growth and competency.

• Ensuring records are maintained according to federal, state, intermediary and agency standards.

• Maintaining confidentiality of patient/family status.

• Assuming responsibilities of secretary and /receptionist duties as needed.


• Licensed Nurse (LVN / LPN) credential

• Graduate from an accredited nursing program

• Impeccable verbal and written communication skills required

• Experience with care planning, reimbursement, and EMR documentation required

• Prior hospice / palliative care experience strongly preferred

• Valid Driver’s License and auto insurance
