Job Duties, including but not limited to:

Ensuring prescription directions for use, dose, medication interactions, therapeutic duplication or overlaps, allergy information, age specific dosage, and other pertinent data are appropriate for individual patients. Intervention with providers shall be made for noted discrepancy of above listed items.

Maintaining controlled substance prescription requirements and dispensing procedures for inpatient orders, discharge prescription orders, and after-hour prescriptions

Conducting patient discharge interviews with appropriate medication counseling (storage, side effects, and dosage, administration, indication, drug-drug and drug-food interactions

Minimum Qualifications:


A Bachelors, Masters, or Doctoral degree in Pharmacy from a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE).


Must have a minimum of one year experience as an outpatient clinical pharmacist for outpatient and minimum of one year experience as an inpatient pharmacist for inpatient.


Must be authorized to work in the US

Must have reliable transportation to and from work site

License/Certification and Training:

Maintain a valid unrestricted current medical license in one of the 50 United States, District of Columbia, Puerto Rico, Guam or U.S. Virgin Islands with ability to obtain clinical privileges

Board certification in one or more of the pharmacy specialties recognized by the Board of Pharmaceutical Specialties, or completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy, or documentation of appropriate education, training, and/or CME in the practice of clinical pharmacy.

Basic Cardiac Life Support (BCLS) through American Heart Association



8-to-12-hour shifts from (anytime 6:00 A.M. - 11:00 P.M.) Sunday to Saturday, including holidays

Requires on-call


8 hour shift between 7:00 A.M. - 6:00 P.M., excluding holidays

No on-call required
